To ensure accurate collection of data, I will reach out directly to collect remaining information on additional campers. You do not need to register seperately for each camper.

Please provide a full list (first & last name) of anyone authorized to pick your child up from camp. Anyone not on this list will not be permitted to pick up your camper.

If you would like to add a pick up password for additional security you may do so here. If a password pick option is selected, even authorized pick up persons will be required to provide the password.

I give permission for my child to be photographed or recorded during Storybook Summer Camp activities. These images or videos may be used for camp-related purposes such as sharing with parents, documenting activities, or occasional promotional use (e.g., website). No names will be shared publicly without additional consent.
